In order to make a payment, please use either the PayPal or Tebra buttons below. You DO NOT need to have a PayPal account to use the PayPal option. However, in order to use the Tebra option, you must have your patient portal login information.

If you want to set up recurring automatic payments using a credit card, debit card, FSA card, or HSA card, please complete this credit card authorization form. Once completed, our team will charge your card for any outstanding balance on your account automatically.

For PayPal:

Simply add a description (name, statement number or other information) to help us correctly apply your payment. On the PayPal screen, make sure you enter the amount that you want to pay. This will start the payment process.

For Tebra:

Log into the patient portal to see your balance and make a payment. If you don't have access to the patient portal, send us a message or call the office for assistance.
